How to Use Wellpath Immediate Care
Wellpath Immediate Care provides 24/7 access to emergency physicians for medical evaluations, in-place care plans, or second opinions in an effort to reduce unnecessary transports to the hospital emergency department.
When your patient has an unscheduled medical problem that may require transport to the emergency room, first contact your local, on-call Wellpath doctor to discuss your patient's condition. Together, you will decide if a Wellpath Immediate Care virtual visit with an emergency medicine physician would be a good alternative.
When you are ready to start a consult, you can do so by visiting wellpath.relymd.app.
Each Wellpath facility has their own login information which has been provided to you. Please use the appropriate credentials to begin the virtual consult. If you have forgotten these credentials, please contact us at wellpath@relymd.com or 919-822-8644.
Step-by-Step Instructions
1. Login with the Wellpath Immediate Care site assigned credentials
2. Always select the default Wellpath Immediate Care facility
3. Add the patient in the search bar
4. Remember to obtain a set of vitals
5. In “Reason for visit” field, type chief complaint and ERMA MRN, and your full name
*entering your name is important to receive credit towards the monthly Top Facilitator Award.
6. Follow the prompts to start the visit
After the Consult
After the consult, prior to closing the application, you will be asked to complete a post-consult survey. This survey is used to understand how your experience with Wellpath Immediate Care and the physicians staffing the platform was perceived, to gain information about common conditions that the service can successfully be used for, and to learn how we can improve our services.
You will be asked:
1. After the telehealth consult did you send the patient to the emergency room?
2. If the telehealth provider had not been available today, my patient would have worsened and gone to the emergency room?
3. If the telehealth provider had not been available today, what would you have recommended your patient to do?
4. Ask patient: The video visit was important in my medical care today.
Patient Care and Follow-Up
The Wellpath Immediate Care physician will document the entire encounter within ERMA. All documentation and and details of the visit will also be communicated to the Wellpath on-call doctor.
Patient notes and care plans, and additional instructions will be made available for review within ERMA.
